New Patient Information
HOW TO BECOME A PATIENT
Please call the office to obtain a new patient application. You can also download and printoff the packet at home using the link below. Completing this new patient packet does several things: It allows us to verify that we participate with your insurance. It also allows us to review which services you are requesting (primary care, mental health services, and/or therapy services) and schedule your appointment appropriately. Once we receive the completed application, we can usually schedule your appointment within 1-2 days. You can fax the new patient packet back to us, mail it by USPS, or email it to carrie@chapmanandassociateshealthcare.com.
INSURANCE INFORMATION
We have worked hard to become enrolled with all insurance companies that serve our community. If we do not participate with your insurance company, we will do our best to investigate joining with your insurer.
PATIENT PORTAL
Once you have established care with us, we encourage you to set up your patient portal through Patient Fusion if you have email. Please ask your provider for more information on this. The patient portal allows you to make, cancel, or reschedule appointments electronically. You can use the patient portal to review your medication list and possibly lab results, depending on which lab you use. You can also message any of the staff using your patient portal-your provider, referral and billing staff. The patient portal is a very convenient way to communicate with us!
